SWEET HOME WORK PERMIT PROCESS

Part of the process requires the applicant to complete the fillable PDF application below. Also, you must have proof of a physical within the last year. If you do not have a current physical on file, you will be required to have your physician email your physical to studentservices@sweethomeschools.org. 

Please fill this out electronically and email it to studentservices@sweethomeschools.org along with an address and phone number where you can be reached if necessary.

Keep in mind, the certificates will be mailed through the US Post Office so allow time for this process which will likely take 3-5 business days. Thank you and be well!

*** IF YOUR PARENT/GUARDIAN IS HAVING DIFFICULTIES SIGNING THE APPLICATION- Have them send email to studentservices@sweethomeschools.org. Simply state: I give permission for my son/daughter to receive working papers.

SWEET HOME WORK PERMIT PROCESS - SUMMER HOURS

Working Papers will be mailed once they have been processed.  If you have any questions, please email studentservices@sweethomeschools.org or call 716-250-1396. 

Applicants are required to complete this fillable PDF application. Also, you must have proof of a physical within the last year. If you do not have a current physical on file, you will be required to have your physician email your physical to studentservices@sweethomeschools.org or fax to 716-250-1363.

Students living in the Sweet Home District attending an alternative school are required to attach a copy of their birth certificate and current physical.
